Tele-Monitoring Equipment Grants Awarded

CCEDC investor, St. Joseph’s Home Health and Hospice will receive a $34,000 state grant to purchase additional remote patient care monitors. The Public Service Commission of Wisconsin grant will purchase home monitoring equipment to assist patients suffering from chronic medical conditions such as diabetes, heart failure, and chronic obstructive pulmonary disease. Using telephone lines, SJH Home Health and Hospice nurses will be able to remotely monitor critical vital signs of patients in their homes. Tele-Monitoring equipment has been demonstrated to prevent avoidable hospitalizations and inappropriate emergency room visits and the equipment is a key part of the HSHS Care Integration strategy to provide high quality, cost-effective and better integrated care beyond hospital walls. The SJH program was chosen because its innovative pilot home monitoring program cares for many patients in rural and underserved areas.
